Your Labs Came Back Abnormal — Here's What to Actually Eat to Get Healthy

I have a version of this conversation multiple times every day.
A patient comes in for a follow-up. Their labs are back. A1c is 7.8 — elevated. LDL is 142 — too high. Blood pressure has been creeping up for two visits in a row.
I explain what the numbers mean. I tell them that diet is one of the most powerful levers we have — that what they eat directly affects every one of these values. I mean it. The evidence behind nutrition for chronic disease management is strong.
And then they ask the question I can never fully answer in the time we have:
"So what should I actually eat to reverse some of these numbers? How can I stay off or reduce my medication burden"
This is the gap that led me to build Rapha. But before I get to that, let me answer the question properly — because most people leave that appointment with a pamphlet and a lot of anxiety, and neither one is actually useful.
Why "eat healthy" isn't an answer
When a physician tells you to improve your diet, they're not withholding information. They're constrained by time and by the reality that personalized nutrition is genuinely complex.
A 20-minute appointment cannot do what a registered dietitian does across weeks of one-on-one sessions. And for many patients, a dietitian referral means a 3-month wait, a copay, and a bill that insurance may not fully cover.
So patients go home. They Google it. They find generic advice — eat more vegetables, cut the sugar, reduce sodium — that doesn't account for their specific diagnoses, their specific lab values, their specific medications, or their actual food budget and preferences.
Generic advice is better than nothing. But it's not what you actually need.
What you need is a plan built around your numbers. Your goals. Your health.
What your labs are actually telling you
Before we talk about food, it helps to understand what you're trying to move and why.
Elevated A1c
A1c reflects your average blood sugar over the past two to three months. When it's elevated — above 5.7% for prediabetes, above 6.5% for diabetes — it means your body is struggling to manage glucose efficiently.
The foods that matter most here are carbohydrates, because carbohydrates break down into glucose. But the story is more nuanced than "eat fewer carbs." The type of carbohydrate matters enormously. Fiber-rich carbohydrates — beans, lentils, whole grains, most vegetables — are digested slowly and produce a gradual glucose response. Refined carbohydrates — white bread, white rice, most crackers and cereals — spike blood sugar fast.
Protein and fat also matter. Including protein and healthy fat at each meal slows gastric emptying, which blunts the glucose spike from whatever carbohydrates you eat alongside them.
High LDL cholesterol
LDL ("bad") cholesterol is primarily driven by dietary saturated fat and, to a lesser extent, dietary cholesterol. Saturated fat is concentrated in red meat, full-fat dairy, butter, and processed foods. Replacing saturated fat with unsaturated fat — olive oil, avocado, nuts, fatty fish — consistently lowers LDL in clinical studies.
Soluble fiber is another powerful lever. It binds to cholesterol in the digestive tract and removes it before it enters circulation. Oats, barley, beans, lentils, apples, and citrus are all good sources. Aiming for 10–25 grams of soluble fiber daily can produce meaningful LDL reductions.
High blood pressure
Sodium is the most direct dietary driver of blood pressure. The American Heart Association recommends under 2,300mg per day — most Americans consume significantly more, largely from processed and restaurant foods. Reading labels matters more than avoiding the salt shaker.
The DASH diet (Dietary Approaches to Stop Hypertension) is the most evidence-backed eating pattern for blood pressure reduction. It emphasizes vegetables, fruits, whole grains, low-fat dairy, and lean proteins while limiting sodium, red meat, and added sugars. In clinical trials, it reduces systolic blood pressure by an average of 5–11 mmHg — comparable to some medications.
Potassium, magnesium, and calcium also play a role in blood pressure regulation. These are found in leafy greens, legumes, nuts, seeds, and dairy.
The problem with managing multiple conditions at once
Most patients with one abnormal lab value have others. Elevated A1c often comes with high triglycerides. High LDL frequently accompanies high blood pressure. Prediabetes and high cholesterol frequently travel together.
The challenge — and this is something the pamphlets never address — is that the optimal diet for each condition overlaps significantly but isn't identical. A very low-carbohydrate diet may help A1c but can raise LDL in some patients. A low-fat approach that helps cholesterol may not adequately address blood sugar if it's high in refined carbohydrates.
Getting the balance right across multiple diagnoses requires knowing your specific numbers and applying guidelines that account for all of them simultaneously. That's not something a general internet search can do for you.
A practical starting framework
While your plan should ultimately be personalized to your specific labs and diagnoses, here are the evidence-based principles that apply broadly across the most common abnormal lab values:
1. Make fiber the centerpiece of every meal. Fiber slows glucose absorption, feeds beneficial gut bacteria, reduces LDL, and helps with satiety. Fill half your plate with non-starchy vegetables at lunch and dinner. Add legumes — beans, lentils, chickpeas — as often as possible.
2. Replace refined grains with whole grains. Swap white rice for brown rice or farro. White bread for whole grain bread with visible seeds. Standard pasta for legume-based pasta or whole wheat. This single swap has measurable effects on both blood sugar and cholesterol.
3. Choose your proteins carefully. Fatty fish (salmon, mackerel, sardines) two to three times per week supports both heart health and reduces inflammation. Chicken and turkey without skin, legumes, and low-fat dairy are good everyday options. Limit red meat to once or twice a week, and avoid processed meats — deli meats, sausage, bacon — as much as possible.
4. Use olive oil as your primary cooking fat. Replacing butter and vegetable shortening with extra-virgin olive oil consistently improves lipid panels. It doesn't need to be expensive — any extra-virgin olive oil works.
5. Read sodium labels, not just the salt shaker. Most dietary sodium comes from packaged and restaurant food, not what you add at the table. Soups, deli meats, condiments, bread, and frozen meals are often high-sodium in ways that aren't obvious. Look for items under 140mg sodium per serving.
6. Distribute carbohydrates across the day. Rather than having most of your carbohydrates at one meal, spread them out. A large carbohydrate load at dinner is harder on blood sugar than the same amount spread across breakfast, lunch, and dinner.
What a dietitian would tell you that a pamphlet can't
A registered dietitian does something that no article can fully replicate: they sit with you, understand your food preferences, your schedule, your cultural background, your cooking skills, and your budget — and they build a plan that fits your actual life.
The most nutritionally perfect meal plan in the world doesn't work if the patient won't follow it. Adherence is the variable that matters most, and adherence depends on the plan feeling realistic and sustainable.
If your physician refers you to a registered dietitian, take that referral seriously. The evidence for medical nutrition therapy — structured dietary counseling with a credentialed professional — is strong across diabetes, cardiovascular disease, and chronic kidney disease.
If access is a barrier — whether due to cost, wait times, or insurance coverage — that's a real problem that affects millions of patients. It's part of why I built Rapha.
The conversation I wish I could have with every patient
Here's what I'd tell you if we had more time in that appointment:
Your lab values are not a verdict. They're data. And unlike most data points in medicine, they're highly responsive to what you eat. A1c can drop meaningfully in three months with consistent dietary changes. LDL responds to dietary fat modification within weeks. Blood pressure responds to sodium reduction and the DASH pattern within days to weeks.
The challenge isn't knowing that diet matters — most people already know that. The challenge is knowing specifically what to eat, given your specific numbers, in a way that's realistic for your life.
